At the submission of this form, you will be assigned an entry number. Please print and save as you will need to label your Dropbox File or DVD with that number. You will also need these numbers to submit with your payment. If you must mail either checks or DVD’s to the Contest Coordinator, please write this number on the check memo or DVD submitted for this entry.
